Step 1: Complete USPS Form 1583

The easiest way to complete the form is by using the USPS Form 1583 Wizard in your PostScan Mail account:

  1. Log in to your account
  2. Go to Settings β†’ Account Settings β†’ USPS Form 1583
  3. Follow the step-by-step prompts to enter your information
  4. Download the completed form for a local notary, or submit it directly to our online notary partner

Step 2: Notarize Your Form

USPS regulations require Form 1583 to be notarized. The notary must:

  • Witness your signature
  • Complete the All-Purpose Acknowledgement section

  • Complete notarization via webcam using our online notary partner
  • Available directly through the Form 1583 Wizard
  • Once notarized online, the form is automatically submitted to PostScan Mail
  • No uploading or mailing required

Local Notary Option

  • Use the wizard to generate a printable form
  • Visit a local notary public to notarize the document

    Upload the following to your account:

    • Signed and notarized USPS Form 1583
    • Color scans of both sides of your Photo ID and Address ID

Step 3: Submit USPS Form 1583 to PostScan Mail

Choose one of the following submission methods:


Option 1: In Person

  • Visit your subscribed location with your completed form and valid ID
  • Some locations may offer on-site notarization (fees may apply)

Option 2: Online Notary

  • Complete and notarize the form through the online wizard
