How do I fill out the 1583 postal consent form?

The easiest and quickest way to complete the Postal Consent Form is using the USPS Form 1583 wizard, which is accessible within your account by going to Settings > Account Settings > USPS Form 1583. Our online wizard will provide a step-by-step guide for filling out this form and will then allow you to download a PDF for a local notary or to submit it directly to our online notary provider.

Notarize Your USPS Form 1583

You will need to have the USPS 1583 Form notarized for us to be able to accept it. You and the notary will need to sign the form and the notary will need to fill out the All-Purpose Acknowledgement.

PostScan Mail offers you the convenience of having your 1583 Form notarized via webcam through our online notary provider, they will also send the notarized form directly to us if you complete your form using our online wizard.

If you filled out the form yourself, please contact us, so we can initiate the online notary transaction for you.

If you choose to find a notary on your own, notaries can be found almost everywhere in the United States. Banks and mail centers will usually have an in-house notary, or you can do a quick web search to find notaries in your area.

Send USPS Form 1583 to PostScan Mail

There are a couple of ways you can forward a copy of the USPS Form 1583 to us.

1. In Person: Visit your subscribed PostScan Mail address and bring the pre-filled USPS Form 1583 form with you along with an ID. Make sure to check the location’s hours or call ahead to ensure they will be available to receive and notarize your form. Some locations may accept the form without a notarization if you come in person and they are able to verify your ID and others will charge a nominal fee to notarize your USPS Form 1583 and then save it in their records.

2. Use Online Notary: If you cannot come in person, then your form requires a notary public to verify your identity. If you choose to use our online notary provider, they will send the notarized form directly to us. Make sure to fill out the USPS Form 1583 wizard within your PostScan Mail account under Settings > Account Settings, the last page will prompt you to send it electronically to our online notary provider who will connect you with a live notary via webcam.

  1. Use Local Notary: Make sure to fill out the USPS Form 1583 wizard within your PostScan Mail account under Settings > Account Settings, the last page will allow you to print the unsigned form and take it to a nearby Notary Public. Once signed, you can return to your Account Settings 1583 page and digitally upload:

    1. Your completed, signed, and notarized Form 1583 (both front and back pages)
    2. A clear, color scan of each side of both your Photo ID and your Address ID.

Change of Address

In order for us to receive your mail, you will need to inform your senders of your new mailing address or you can file a Change-of-Address order with the USPS. The Change-of-Address process only works in the US and it will only forward your mail for up to 1 year, you will ultimately still have to ensure all of your senders know of your new PostScan mailing address.

If you are subscribing to an address outside of the US or if you are not currently receiving your mail at a US address, you will not be able to submit a Change-of-Address order and you will simply need to inform your senders of your new mailing address.

The process of filing a Change-of-Address order is straightforward and can be completed by choosing one of the following options:

  1. Submit a Change-of-Address form online with the USPS, you can do so by visiting the Official USPS Change-of-Address website.
  2. Inform your senders of your new PostScan Mail mailing address
